About this Session
In this first session of the Pathways to Possibilities webinar series, we’ll explore foundational resources that support quality transition planning across stakeholder groups. From assessment tools to interagency collaboration frameworks, we’ll walk through how educators, VR staff, families, youth, and others are applying these tools to improve outcomes.
Training Objectives:
- Identify key foundational NTACT:C resources that support effective secondary transition planning for youth with disabilities.
- Describe how various stakeholder groups—including educators, VR professionals, families, and youth—can use these resources to promote informed planning and self-determination.
- Analyze how the NTACT:C tools foster interagency collaboration and shared responsibility in transition planning.
- Recognize how to access and navigate the NTACT:C website to locate essential resources aligned with each stakeholder’s role in the transition process.
